Wait for the schedule of Faculty Evaluation to be announced.

Accomplish the Faculty Evaluation Form sent via the Google Form link provided by Guidance.

Submit your accomplished evaluation.

Wait for the summary of Faculty Evaluation ratings to be prepared and signed.

Receive the Certificate of Evaluation, if issued for your participation.

Mahahalagang tala
- Evaluation is conducted only when scheduled by Guidance, once per semester. The Google Form link is sent by Guidance only when an evaluation is scheduled; it is not a standing, always-open application channel, and an old link should not be treated as currently valid.
- Fees and processing times are attributed to the 2026 Citizen's Charter, 2nd Edition; they have not been separately reconfirmed by a newer source.
