Authorization Letter from the owner or company, or Board Resolution/Secretary's Certificate for a corporation

Photocopy of the owner's and representative's IDs. Accepted identification includes UMID, Driver's License, PRC ID, Passport, Senior Citizen ID, SSS ID, TIN ID, PWD ID, Solo Parent ID, Postal ID, School ID, Company ID, or Government Agency ID if no other government-issued ID is available
Saan kukunin: Owner or representative; issuing government agency, school, or company

Submit the request at the Business License and Permit Division, Windows 61 or 62.

Pay the corresponding taxes and fees at the City Treasurer's Office, Windows 5-10, and receive the Official Receipt.

Give the Official Receipt to BLPD, Windows 61 or 62, and wait while staff prepare the certification or request. Drop the Feedback Form (CSM) at the Suggestion Box.
