Saan kukunin: Hospital or doctor

Naaangkop lamang: If unavailable, submit a Marriage Contract, Baptismal or Confirmation Certificate, or Birth Certificate of the client's children
Live Birth Certificate
Saan kukunin: Philippine Statistics Authority

Notarized Barangay Resolution certifying that the prospective recipient is Kapampangan and a bona fide barangay resident, or a migrant with at least 20 years of permanent residency in Pampanga at the time of the grant

Visit the CSWDO, obtain a queue number, wait for your turn, and ask about the service requirements.

Participate in the assessment interview, answer the questions, and wait for the decision. A home visit may be conducted if necessary.

Upon approval, receive the Social Case Study and sign the logbook to acknowledge its release.
