Saan kukunin: Respective Barangay

Certificate or Affidavit of Guardianship (1 original)

School ID of the client (original for presentation and 1 photocopy)

Photocopy of Voter's ID, Voter's Certification, or another accepted valid ID with specimen signature of the parent or guardian

Paano mag-apply

Visit the CSWDO, obtain a queue number, wait for your turn, and ask about the service requirements.

Participate in the assessment interview, answer the questions, and wait for the decision.

Upon approval, receive the Social Case Study and sign the logbook to acknowledge its release.
