   Barangay Indigency of the student and parent or guardian (1 original)

   Saan kukunin: Respective Barangay

   Barangay Certificate of Residency of the student and parent (1 original)

   Saan kukunin: Respective Barangay

   Naaangkop lamang: Submit one accepted identification document

   Photocopy of Voter's ID, Voter's Certification, or another accepted valid ID with specimen signature

   Saan kukunin: Requesting client or issuing agency

   Naaangkop lamang: For guardians

   Certificate or Affidavit of Guardianship (1 original)

   Saan kukunin: Respective Barangay or Legal Office

   Registration or Admission Confirmation or Certificate of Enrollment of the student

   Saan kukunin: School, university, or college

   Naaangkop lamang: Submit one applicable proof of income

   Proof of income, such as a payslip, Income Tax Return, Certificate of Tax Exemption, bank statement, or affidavit of no or low income (original for presentation and 1 photocopy)

   Saan kukunin: Employer, Bureau of Internal Revenue, respective bank, or notarial firm

   Proof of billing, such as an electric or water bill
